What you should know about receiving care through telehealth — and your right to choose otherwise.
The short version
Please read this Consent carefully. By using Wellday's telehealth services, you confirm you have read and agreed to it.
Wellday delivers your migraine care through telehealth — video, messaging, and online tools rather than in-person visits. There are real benefits (convenience, access, no waitlists) and real limitations (no physical exam, technology can fail, not every condition can be safely treated remotely).
You can withdraw your consent any time. Telehealth is not for emergencies. If you need urgent medical attention, call 911 or go to an emergency room.

What telehealth is
Telehealth is the delivery of healthcare services using electronic communications — video calls, secure messaging, online forms — between you and a licensed clinician in a different physical location.
For Wellday, telehealth includes reviewing your intake, video consultations, asynchronous messaging with your care team, and electronic coordination with pharmacies and insurers.
The clinician providing your care is licensed in the state where you are located and practices through Wellday Medical Affiliates.

Benefits of telehealth
Easier access to care without travel. Faster appointments — no specialist waitlists. Care from home, work, or anywhere with internet access. Reduced costs compared to traditional specialty care. Ongoing care team messaging between visits.

Risks and limitations of telehealth
No physical examination. Your clinician cannot perform a hands-on exam and relies on the information you provide. Some conditions can't be fully evaluated through telehealth.
Information dependency. Care quality depends on the accuracy of what you share. Incomplete information may lead to incorrect assessments or missed diagnoses.
Technology limitations. Internet connections, cameras, and software can fail.
Privacy considerations. Even with HIPAA-compliant technology, electronic communications carry risks. You're responsible for conducting telehealth in private locations.
Data security. No electronic system is perfectly secure. There's some risk of unauthorized access.
Not appropriate for all conditions. Your clinician may decide your condition needs in-person evaluation and refer you accordingly.
Not for emergencies. If you're experiencing a medical emergency, call 911.
Communication delays. Messages may not be answered immediately. Don't use the platform for urgent concerns.

Alternatives to telehealth
You can choose in-person care with your primary care provider, a neurologist or headache specialist, an urgent care or emergency room, or other telehealth providers. You can discuss alternatives with your clinician anytime.

Your rights
You have the right to:
- Withdraw this Consent anytime in writing by emailing privacy@welldayhealth.com
- Refuse telehealth services anytime without affecting other care
- Access your medical records related to telehealth encounters
- Request that any consultation be terminated at any point
- Be told the identity, credentials, and location of your clinician
- Receive a copy of this Consent

Privacy and confidentiality
Your telehealth consultations are conducted using HIPAA-compliant technology. Your health information is handled by Wellday Medical Affiliates as covered entities under HIPAA, and is protected accordingly. Wellday operates as a Business Associate when handling health information on their behalf. See our Privacy Policy for details.
You agree to conduct consultations in a private location and not to record consultations without your clinician's express consent.

Emergencies
Telehealth is not for emergencies. Call 911 or go to your nearest emergency room if you experience:
- Sudden, severe headache unlike any you've had before
- Headache with fever, stiff neck, confusion, vision loss, weakness, or difficulty speaking
- Headache after a head injury
- Any other situation requiring immediate medical attention

Costs
Wellday's services are direct-pay and not billed to insurance. Your $119 consultation fee covers the visit and care plan. Subscription fees cover ongoing care. Medication costs are separate. See our Subscription & Refund Policy.

Consent
By using Wellday's telehealth services, I confirm:
- I have read and understand this Consent
- I understand the benefits, risks, and limitations of telehealth
- I consent to receiving healthcare services through telehealth on the Wellday platform
- I understand I can withdraw consent anytime
- I understand telehealth is not a substitute for emergency care
- I authorize my clinician to share information about my care with other providers as needed
- I am at least 18 years of age and located in a U.S. state where Wellday operates
- The information I provide is accurate to the best of my knowledge
